背景
乳がん検診では、受診者が高齢になるにつれ、検診による早期のがん発見というベネフィットと、過剰診断という害のプロフィルが逆転すると考えられており、多くの国では対策型乳がん検診に年齢上限が設けられている。
アメリカYale School of MedicineのRichmanらは、SEER（Surveillance, Epidemiology, and End Results）プログラムにリンクされたfee-for-service診療報酬請求に基づき、最近検診を受けた70歳以上の女性（n=54,635）が、検診を継続した場合と継続しなかった場合の乳がん診断・乳がん死亡リスクを測定し、年齢別の過剰診断の割合を推定する後向コホート研究を実施した。
