特集関節疾患の画像診断：変形性関節症（OA）と関節リウマチ（RA）存はX線における関節破壊進行の強い予測因子である3）とともに早期再発の予測因子である12）と報告されている。したがって、特に早期RAでは関節超音波による画像的寛解を目指した治療を行うべきである。関節超音波には前述以外にも副次的な利益が期待できる。まず、画像所見を臨床所見にフィードバックすることより関節の診察技術が向上し、さらにRA病態のより深い理解につながる。また、活動性滑膜炎や関節破壊を視覚的に捉えることは、患者状態の正確な把握のみならず、患者教育および治療方針相談の場となりうる。検者および被検者の双方において満足度が高い検査といえる。RA患者が身体機能障害を来さないためには、早期に診断し、適切な活動生評価のもと寛解を目指した積極的な治療が必要である。そのすべてのプロセスにおいて関節超音波は非常に有用である。さらに、比較的安価で被曝の心配もなく繰り返し施行できるという利便性の高さも魅力である。しかし、冒頭にも述べたように検者の技量に左右されるため、手技や評価法の標準化を進める必要がある。関節超音波が、リウマチ医（内科・整形外科）、放射線科医および超音波技師の共通の有用な検査ツールとしてより実用的に普及することを期待したい。参考文献1） Wakefield RJ et al: Should oligoarthritis be reclassi-fied?
