34映像情報メディカル 2014年1月などの臨床的疾患活動性の指標をもとに、その設定した目標を3ヵ月で達成するために、合成DMARDsと生物学的製剤を組み合わせた抗リウマチ治療の提言である。目標は臨床的寛解の導入と維持であり、これらが達成された場合は、RAの機能予後、関節予後および生命予後は改善する。特に前2者に関しては罹病期間の長短でのエビデンスが集積され、罹病期間が短い症例（早期RA）が、より恩恵を受けやすい12）。また、臨床的疾患活動性に加え、RAの関節予後や抗リウマチ治療の反応性を評価する時にも、MRIや超音波は有用である5）。すなわち、MRIでの骨炎、MRIや超音波での滑膜炎は関節破壊の予後指標であり、これら画像で検出される炎症は、治療反応性の予測や経過を観察するのに重要であることが明らかとなってきた（表2）5）。RAの早期診断は、臨床分類（診断）基準である2010 RA分類基準にMRIや超音波画像を組み合わせることで、より的確に行えるようになってきた。診断後の抗リウマチ治療の導入も、EULARレコメンデーション：2013改訂版にあるように、エビデンスにもとづき、寛解を目指す治療が浸透してきたが、この治療経過を評価する場合にもMRIや超音波から得られた画像情報の有用性が明らかとなってきた。罹病期間が長いRAと比較して、早期RAは、早期からの治療が奏功すると臨床的寛解、機能的寛解、構造的寛解に加え、画像的寛解（MRIや超音波でも炎症所見がない状態）が達成されやすいとされるが、今後は前向き研究における実証が期待される。参考文献1） Aletaha D et al: 2010 Rheumatoid arthritis classifica-tion criteria: An American College of Rheumatolo-gy/European League Against Rheumatism collabo-rative initiative.
